
常聽到很多新手媽媽抱怨抱孩子抱到有「媽媽手」,很多老手媽媽抱怨做家事做到「媽媽手」。
到底什麼是「媽媽手」呢?
>好發族群
媽媽手其實是大拇指的肌腱發炎造成的疼痛和不適,因此並非限定只發生在媽媽身上,只要生活中常用拇指施力都可能是媽媽手的高危險群。
像是抱小孩、端盤子、拿重物,長時間握筆寫字等活動,都會讓拇指受力過多,也就有可能引起發炎。
另外,反覆的拇指和手腕關節活動,如切菜、滑手機、用滑鼠,也容易造成「媽媽手」。
>症狀及病因
過度的使用拇指和手腕造成外展拇長肌和伸拇短肌的肌腱發炎,使得拇指靠近手腕處會疼痛、腫脹,甚至影響到大拇指的動作。
>檢測方式
1.將大拇指包在四指裡握拳
2.將握拳的手在小指的方向彎,若拇指靠近手腕處會疼痛,則可能患有「媽媽手」
>治療方法
1.穿戴副木--職能治療師會依據患者的手型量身打造副木,穿戴後可將大拇指固定於適當位置且限制其動作,有利於減緩疼痛和促進組織的修復。
2.物理治療--透過電療、熱敷、超音波治療等可以減緩不適。
3.藥物治療--分為口服和外用,主要功能為消炎止痛。
4.局部注射--局部注射消炎藥物。
5.手術治療--若經過保守治療仍無法改善,嚴重的話需進行手術治療。
>預防方法
1.避免讓單一關節承受過多的壓力,例如抱小孩時可以五指併攏且保持手腕是平的,用手臂和五根手指平均受力。
2.若從事的活動一定要使用拇指ex.寫字、滑手機,則需要適度的休息,並搭配手指手腕的伸展運動。
3.平時可使用握力器、拉橡皮筋、手指撐開橡皮筋等活動加強手指肌力,但若已經有疼痛和不適者應先休息,避免過度運動造成狀況惡化。
「媽媽手」多半是長期姿勢不良、過度使用造成的,因此若有上述的症狀記得多休息並留意自己的動作,若疼痛越趨明顯建議及早就醫治療。
Mommy thumb (De Quervain's syndrome) is quite common in new mothers carrying their babies and housewives doing domestic works.
So, what is it exactly?
> Risk factors
De Quervain's disease is a tenosynovitis of the thumb which not only occurs in mother group but those overusing their thumb in daily tasks. Activities like carry baby, pick plates, lift heavy items and writing will put strain on thumb and induce the inflammation. In addition, repeatedly using of thumb and wrist such as cutting, phubbing and using mouse may develop de Quervain syndrome as well.
> Symptoms and cause
Pain, swollen and movement limitation over the thumb and wrist is due to the inflammation of the tendon of extensor pollicis brevis and abductor pollicis longus.
> Physical examination
1. Grasp the thumb with the other four fingers. 2. Deviate the hand toward ulnar side. If sharp pain occurs, de Quervain is likely the cause.
> Treatment
1. Splint- Occupational therapist make a tailored splint to position and immobilize the thumb which will reduce pain and facilitate tissue recovery.
2. Physiotherapy- treatment like TENS, heat, ultrasound and etc. to ease discomfort.
3. Medication- internal and external use to relieve inflammation and alleviate pain.
4. Injection- inject anti-inflammatory medication
5. Surgery- If no improvement after conservative treatment, may consider undergo surgery.
> Prevention
1. To avoid stress on one joint only. For example, lifting baby with five fingers close and maintaining wrist in neutral position are ways to even the stress on the arm and hand.
2. Have enough rest and stretching exercise after repeatedly using thumb (writing and slide phone)
3. Use the hand grip and rubber band to strengthen grasping power. Rest if feel pain and discomfort, prevent deterioration after excessive exercise.
De Quervain’s syndrome is usually caused by poor posture and overusing. Remember to rest and watch out your own movement if you have above symptoms. Do seek medical advice if pain increases.
