
「被動關節活動」是職能治療師在臨床上常用的介入手法,雖然和按摩一樣都是直接的肢體接觸,但它的功用和手法都沒按摩不太一樣,今天就來和大家介紹這個容易讓許多民眾混淆的治療方法!
被動關節活動
>主要目的:
1) 避免關節僵硬和軟組織攣縮
2) 促進血液循環,減緩肢體的疼痛及不適
>適用對象:
肢體無主動動作或者主動動作無法做到最大角度
>執行方法:
每個關節都有特定的動作,需盡可能做到最大角度
>執行頻率:
每個關節每次做10下,可在最大角度停留10秒
每天三回,視個案狀況不同可做適當增減
(但可以確定的是只在治療室做一次是不夠的!!!!)
綜合上述,被動關節活動能預防長時間不動造成的關節僵硬和疼痛,但僅僅在治療室由治療師做是不夠的,真正有效的方式應該是治療師教導照顧者或個案本身執行被動關節活動的技巧,由照顧者和個案本身在家中自行執行,如此才能達到足夠的頻率,也較能有效的運用治療時間。
Passive range of motion (PROM) is often used by OT in clinical setting. Though the direct contact to skin makes it similar to massage, the manner and effect are actually different. Let me explain this confusing treatment!
The main goal-
1. To prevent joints stiffness and soft tissue contracture
2. Facilitate circulation and relief limbs discomfort Ideal candidates-No active movement or unable to do maximum range
Techniques-
Each joints have their own specific movement, possibly achieve maximum ROM
Frequency-
10 times per joints and sustain the maximum range for 10 secs/ 3 times per day. The amounts depend on clients’ condition. (But 1 session in the treatment room is surely not enough)
In conclusion, PROM can prevent joints contracture and pain caused by long time immobilization. However, merely done by OT in treatment session is definitely insufficient. The truly effective way is caregiver education or clients’ initiation, only initiatively done by caregiver and clients themselves at home can reach adequate doses as well as utilize treatment time efficiently.
